1:30pm - 4:30pm - Symposium Pre-Session: Integrating Social Determinants of Health Into the Care Model Through Community Partnerships
Explore how health systems (their providers and networks) and community benefit organizations can collaborate to address social determinants of health (SDOHs). The session will delve into both the clinical and operational approaches that communities from across the country are taking to address policy and systems change, provider challenges, patient needs and best practices. Attendees will gain broad knowledge as well as tactical approaches that can be implemented locally.
1:30pm - 4:30pm - Symposium Pre-Session: Effectively Working Across the Care Continuum
Explore how patient care that is well-coordinated across the continuum of care from with a focus on using care guidelines to plan effectively, leverage a breadth of post-acute opportunities including transitions of care and home health programs. Attendees will gain deeper insight into ways to think about the use of standardized workflows, clinical guidelines and high quality post acute programs to improve outcomes, patient/family experiences, and reduce total costs of care.
1:30pm - 4:30pm - Symposium Pre-Session: Implementing a Next Generation Data and Analytics Strategy in Health Care
Explore the approach that the Prisma Health team and its partners have taken to develop a modern data and analytics strategy and infrastructure. Attendees will gain an understanding of key considerations in modernizing data and analytics across a health system and engage in discussion around the challenges, expectations, and opportunities analytics hold in health care.
5:00pm - 6:30pm - HSC Research Showcase & Welcome Reception
The Health Sciences Center Research Showcase will profile innovative health care research projects led by investigators from Prisma Health and Health Sciences Center academic partners: Clemson University, Furman University, and University of South Carolina. Investigators will be in attendance at poster displays to discuss research projects and exchange ideas. Featured projects will be of high relevance to Symposium attendees, focused on changing models of care, addressing social determinants of health, improving processes and clinical outcomes, negotiating changes in policies and payments, and preparing the workforce of the future.
